    • Proof of 2 Vaccines or Positive Titer showing immunity
    • Annual Respirator fit test. Locations that allow onsite testing: a number of medical centers.
    • Flu Vaccine or declination required from October 1 - May 31. Declinations not accepted at certain medical centers.
    • TB Test - Declinations not accepted at certain medical centers.
    • Physical/Health Statement - must be done annually, except at certain medical centers where it must be done within 60 days of start and must state clear to work.
    • National Criminal Background Check within one year of start date. Additional specific requirements for certain states.

    • Must score 80% or higher except in Georgia at SRGA must score 85% or higher.
